This event is MANDATORY for all cheer & football registrants. 




Each division will have a designated time to show (will be emailed closer to date). It is IMPORTANT your participant stick to the time of their division so we can move you through this event as quickly and efficiently as possible.

Physical Requirements

Physicals Checklist: 

- Is the form dated after April 1st, 2024?
 - Did the doctor sign AND stamp the form?
-  If the form is from Kaiser, include all of the pages + doctor sign AND stamp.

If you answer " No" to any, FIX IT before turning in! We will not accept any physical form that is not complete.

Without a physical, your
participant CANNOT set foot on an AJT field/practice. 

School Enrollment

This is for proof of AGE AND GRADE : 

 - Does the form include: 
child's name
date of birth
address
* grade for the 2024-2025 school year  

 - The information can be typed on school letterhead. 
The school must sign and stamp the letter and include:
child's name
date of birth
*grade for the 2024-2025 school year.


* Proof of grade is REQUIRED for ANY PARTICIPANT who is 12+ years of age and older.

Deposits

 

All items are due at Document Turn In 07/07/2024

If the deposit checks/ fundraiser are not provided, your registration is not complete and your participant cannot step onto an AJT practice field or event until the next submission date. 



1. Mandatory
  Fundraiser Raffle Football & Cheer 
$50.00
This can be paid by check, cash, or credit card 
 If you have multiple children in the program, a separate check is required for each child
This only applies for max two (2) participants. 
( If you have two participants, you will owe $ 100.00. If you have three (3) + participants, you owe $ 100.00 and  get two raffle books)

This raffle will be held at or Homecoming Game (last home  game in September 28, 2024).
You will get a raffle booklet with five (5) tickets that you can sell or keep in September. 
Only these tickets will be allowed for entry in our "Big Ticket Items" and trust us, these will be WORTH IT! 





Deposits are accepted via check/ money order/ cashiers check ONLY
If you have multiple children in the program, a separate check is required for each child.


Volunteer Deposit Check  -Cheer & Football 

1. Volunteer Deposit Check
$200
- If you have multiple children in the program, a separate check is required for each child. 
The volunteer deposit check is collected prior to the start of the season.
- Checks must be made out to:  AJT
- Add your child's name and team to the memo line (i.e., Patty Smith 8u Cheer)
* Checks are NOT deposited unless you FAIL to complete your required 10 hours/ participant  (max 20 hours). 



Equipment - Football : 

2. Football Equipment Deposit Check
$250

This applies to ALL football players, regardless if you have your own shoulder pads and/or helmet. 
The equipment deposit covers all equipment that is provided which includes 2 uniform pants, shoulder pads and helmet.
- If you have multiple children in the football program, a separate check is required for each child.
- Checks must be made out to:  AJT
- Add your child's name and team to the memo line (i.e., Patrick Smith 8u FB)
* Checks are NOT deposited unless you FAIL to return your loaned equipment
